STORY HIGHLIGHTS

Helping Lead the Way in Enhancing Patient-Centered Care and Clinical Services

Mary Geloso, RPh from The Medicine Shoppe in Ilion, NY, has embraced the opportunity with the Flip the Pharmacy (FtP) program by implementing a med sync program into their everyday practice. This change in the workflow has helped identify non-compliant patients and those falling off to now have better tracking through a streamlined approach. A total of 58 new patients were enrolled in their sync program which was identified based on compliance last year. A future goal is to involve more pharmacy staff, and Mary and her team plan to grow the number of eCare plans to continue improving patient care and outcomes. She and 13 other CPESN NY pharmacies in UNY and WNY are part of the FtP program and is helping drive change and advancement with clinical services in the pharmacy setting.

The Medicine Shoppe is also pursuing DSMES accreditation to offer a new diabetes management program to their patients and is teaming up a pharmacist with a nurse to help lead the program.

“Having an RN and a pharmacist offers a unique, well-rounded team approach that brings expanded clinical experiences to help better educate our DSMES patients,” says Steve Pardi, owner of The Medicine Shoppe.

This innovative mindset is just another example of how community pharmacy continues to transform its crucial role in the healthcare profession.
