CPESN NY, LLC Pharmacies Growing as a Viable Option for Accessible Diabetes Education and Programs

Our network’s most established program, Middleport Family Health Center (Middleport, NY) has been ADCES accredited since 2013. During this time they have provided diabetes education and support services to over 100 patients. This dedicated program has been valuable in the community and is well-received by local medical providers. They are one of the premier community pharmacy residency programs in New York State that utilizes this program to train future pharmacist diabetes educators. “We have had the opportunity to create a collaborative agreement with one of our local primary care offices where we provide education and make recommendations for adjustments to medication regimens for diabetes patients, further extending our reach and impact in the care of our patients,” says Lindsey Landi, PharmD, Residency Director at Middleport Family Health Center.
